Job description:

Our Case Managers collaborate with our internal treatment team and community agencies to directly advocate for at-risk children and families, providing important needs assessments, prevention and intervention services, and progress monitoring.

Here's what you can expect in the way of specific job responsibilities and time allocation estimates:

Direct Service – Direct contact, indirect contact, and brokered services (50%)

• Assist clients in achieving their goals
• Broker support to clients
• Provide transportation to clients associated with specific goals
• Attend court with clients and
• Respond to crisis (depending upon referral type)
• Develop relationships with agencies in the community
• Work with other social welfare agencies
• Refer to community agencies

Reporting – Report writing and information entry (50%)

• Email referring agent and supervisor following each contact or attempted contact
• Review and upload monthly reports to Kidtraks and NYTD
• Maintain case and agency paperwork according to standards
• Prepare written reports for the Department of Child Services / Probation office within identified timeframes

Job Requirements:

• A bachelor's degree in social work or related field is required.
• You must be at least 21 years of age
• You have a valid driver's license and insurance with 6 or less points on driving record.
• Knowledgeable regarding social service systems within the state
• Knowledge of and adherence to current confidentiality practices
• Appreciation for and ability to respond to cultural differences
• Maintain relevant networks of important contacts
