Missouri Highlands Health Care is seeking to hire a Case Manager for Doniphan Urgent Care.

The Case Manager will be directly supervised by the Clinic Manager.

II. GENERAL DESCRIPTION

The case manager is responsible for providing care coordination and health education for low risk, at risk, and socially high-risk patients who receive primary, dental and/or behavioral health care at Missouri Highlands Health Care, including, but not limited to eligibility assistance, women’s health services, and transportation assistance.

III. MINIMUM QUALIFICATIONS

· Obtain a license as an LPN or a certification as a medical assistant or medical tech.

· Two years’ experience in an ambulatory setting with at-risk patients preferred.

· Knowledge of available community resources and the ability to evaluate resources and make appropriate referrals.

· Knowledge of health-related financial assistance, including Medicare and Medicaid.

· Ability to interpret and comply with federal, state, and local laws and regulations.

· Ability to deal effectively with a diverse patient population.

· Ability to prepare reports and communicates effectively orally and in writing.

· Previous experience in operation of office machinery: personal computer, copier, FAX, postage machine, printers.

· Adequate vision. Ability to reach, stoop, walk, and lift 25 pounds.

IV. ESSENTIAL JOB DUTIES AND RESPONSIBILITIES

Provide the following case management services in accordance with the policies and procedures:

1. Assessment: Provide an assessment of individual/family needs and strengths and develop and monitor a plan for assistance.

2. Interventions: The Case Manager will provide direction, support, guidance, education, and training for the recipient of services. The Case Manager will

provide advocacy, information, referral, and follow-up services to verify the promotion of the welfare of the patient.

3. Documentation: The Case Manager will maintain current documentation for the assigned cases according to policies and procedures, identifying the service delivery and progress toward outcomes.

4. Assist with Delta Regional and Show Me Health Women program, and other programs as needed.

5. Assemble and maintain charts and files for case management.

6. Maintain daily, monthly, quarterly, and annual paperwork requirements.

7. Make referrals to community agencies on behalf of patient, if applicable.

8. Gather and update resource information.

9. Collaborate with other provider to offer services such as support groups, educations experiences, etc.

10. Adapt to change as needed.

Job Type: Full-time

Pay: $13.36 - $16.00 per hour
