Case Manager Job at Sandstone Healthcare Group
Sandstone Healthcare Group is seeking a skilled and dedicated Case Manager to join our team in Salt Lake City, Utah. As a Case Manager, you will be responsible for coordinating care and providing support to our patients and their families. LPN and case management experience is preferred, with skilled nursing experience as a plus.
Responsibilities:
- Assess patient needs and develop care plans
- Coordinate care with physicians, nurses, and other healthcare professionals
- Communicate with patients and their families about care plans and progress
- Monitor patient progress and adjust care plans as needed
- Ensure compliance with regulatory standards and policies
- Manage documentation and maintain accurate records
Requirements:
- LPN license and case management experience preferred
- Skilled nursing experience a plus
- Strong communication and organizational skills
- Ability to work independently and as part of a team
- Knowledge of regulatory standards and policies
