Case Manager - School Based Services
- Position Title: Case Manager
Reports to: Project Coordinator
General Description: The primary responsibility of the Case Manager is to provide support, guidance and resources for students in local school settings. This includes participating in daily screening activities at assigned school sites and working closely with other Wellness Program and school staff. Case Manager will be responsible for effectively communicating with parents, clinicians, students as well as school staff. Flexibility and ability to travel within school systems is a must.
DUTIES/RESPONSIBILITIES
- Administer Wellness Screening to assigned students/campus
- Respond and communicate to school staff and parent questions about Wellness Program
- Review Wellness Screening data for follow up with students
- Provide Risk Assessment follow ups
- Provide case management according to Wellness Program procedures in connecting families to resources that are recommended by Wellness Program clinician(s)
- Document case notes within Data Management System
- Participate in Weekly Consultations, Monthly Staff Meetings and other meetings as appropriate to position
- Follow safety protocol for at-risk and safety situations, reporting when necessary.
- Maintain confidentiality with students, family, and staff

- Assertive and direct communication skills
- Well-developed listening skills
- Excellent organization skills with attention to detail
- Well versed in the Wellness Program policies, procedures and best practices
- Well versed in the mental health resources available in the community
- Ability to communicate difficult information in a confident and compassionate manner
- Persistent follow through to see that families are connected to recommended resources
- Ability to problem solve in the moment and perform at an elevated level of independence
- Ability to remain calm during times of crisis, and follow proper protocol for urgent evaluations
- Knowledgeable of local resources
Qualifications
Bachelor’s degree required: Examples but not limited to Social Services including Social Work, Psychology, Education and other Counseling related fields of study.
