Banyan Treatment Centers, is currently recruiting for full-time a full-time Case Manager to join our team in Lake Worth, Florida.

The Case Manager is an extension of the Clinical team as he/she diligently works with clients in managing their outside affairs, solidifying an aftercare plan and preparing the client for discharge. This position reports directly to the Clinical Director.

Schedule Available:

• Full-time schedule (40/hrs per week); scheduled days to be discussed in interview.

Primary Job Responsibilities:

• Review assessments from referring facilities and note problems/issues that require further attention
• Contact referral source and other key individuals to gather additional information as necessary
• During first session with patient, review initial treatment plan, including case management issues and be sure that

patient and staff assignments and responsibilities are understood and in progress * Formulate a comprehensive treatment plan with patient that effectively addresses patient issues, as it pertains to continuing care, and is completed within established time frames

• Document the patient’s progress accurately as it relates to case management
• Continue treatment planning process throughout patient’s stay in treatment, as it pertains to medical, legal, and employment issues, opening new problems and developing or revising treatment plans as necessary to assist in continuing care.
• Maintain regular contact and open communication with families and assist them in finding resources to help themselves
• Link patient with internal and external resources as needed to meet patient needs

patient’s progress in recovery Use good clinical judgment in moving patient through treatment levels Prepare patient to maintain recovery post-discharge Develop a discharge plan that meets patient needs for ongoing support

Qualifications:

• Associate's Degree and/or meets experience requirement; 1-3 years of work experience in the field of substance abuse or mental health.
• Strong communication skill, both written and verbal, is required.
• KIPU and Salesforce experience is highly regarded but not required.
