Adapt Integrated Health Care is currently recruiting a full time Behavioral Health Case Manager for Curry County! This position will be based in Brookings, but will float between our Gold Beach, Port Orford, and Brookings offices.

Job Description:

Provides case management and providing skills training groups.

Job Duties:

• Collaborates with the client on a needs assessment to identify areas of focus.
• Assists with recovery plan development and implementation, coordinating with the primary counselor and other key staff.
• Investigates resources and provides referral to appropriate community agencies.
• Integrates clients supports (family, social, community) to help remove barriers for clients.
• Coordinates referrals within the ASAM level of care to residential facilities to effect a smooth transition between services.
• Documents in a timely manner each intervention utilizing appropriate forms and coordinates information with the clients support team.
• Provide Skills Training Groups
• Other duties as assigned.

Qualifications:

• High School Diploma or equivalent required
• Completing MHACBO minimum training and experience requirements for QMHA
