Case Management Job at TWIN OAKS JUVENILE DEVELOPMENT
The Dove Academy is currently seeking a case manager to work with girls in a life-changing residential program that provides wisdom, hope and encouragement. The goal of this program is to make a lasting impact on these young ladies while providing safety and stability in a nurturing therapeutic community.
Case Manager Job Summary & Requirements:
- Provide care to meet the physical, emotional, and social needs of the youth in the group home setting.
- Conduct intake and program orientation procedures with each new youth in accordance with program policy
- Provide a trauma informed approach and consider each child’s needs and trauma history.
- Initiate a written service plan/treatment plan and update as required.
- Develop a treatment plan which will be reviewed every 30 days and updated every six months.
- Effectively implement the behavior motivation system and address unacceptable behavior.
- Interact with students appropriately while in vocational, educational, work and leisure settings.
- Prevent and intervene in youth altercations and incidents and effectively communicate student information and concerns with other employees.
- Maintain all youth information in confidentiality and complete all documentation in a timely manner and according to program policy and procedures.
- Current state driver’s license
- Bachelor's Degree in a related human service field and 2 years case management experience.
- Meet criminal background check and drug screening per state requirements.
