Tri County Care Management Organization (CMO) is a non-profit organization that provides care management (case management) services for children with complex mental health and/or behavioral needs, as well as developmental/intellectual or substance use challenges in Hunterdon, Somerset, and Warren counties. We partner with families and communities to help youth succeed at home, in school, and in the community.

We are currently seeking a Care Manager to provide care management (case management) services based out of our Branchburg, NJ office and serving Somerset and Hunterdon Counties.


Responsibilities Include:


  • Provide intensive wraparound/case management services to families and children with mental health and/or behavioral needs, as well as developmental/intellectual or substance use challenges.

  • Advocate and help find resources for youth to access formal and informal services and be a liaison for care services.

  • Facilitate/attend meetings at homes, schools, and other services with the youth and families.

  • Develop, document, and follow up on plans of care using an electronic health record system.

  • Bachelor’s degree in social work, psychology, or related discipline.

  • At least one (1) year of experience in a child service system environment.

  • A valid driver's license in good standing and a vehicle able to be used for business purposes are required.

  • Experience with DD/ID population preferred.

  • Strong computer skills including MS Office Suite and ability to learn job-specific computer programs.

  • Ability to adapt to different work environments including, fieldwork, office, and virtual settings.

  • Strong interpersonal and organizational skills are required.
