The
CARE Coordinator is responsible for a high call volume of inbound and outbound calls while providing exceptional customer service. Responsible for accurate data entry that affects the results of day-to-day patient care delivered to Hospice of Michigan’s (HOM) patients and families. The CARE Coordinator schedules patient’s enrollment visits using critical thinking skills to utilize the Enrollment Nurses to their fullest while using minimal resources.
The CARE Coordinator will explain Hospice services to patients, families, caregivers and patient care facility staff members. Schedules necessary appointment(s) required for patients and families to complete the sign-on and/or admission. Also will accurately enter appropriate information into electronic documentation system(s) in a timely manner, including insurance information. Quite often will respond to crisis calls and will take appropriate action to resolve the situation for best positive outcome. Initiates follow-up calls to ensure patient and family knowledge of planned visit schedules. Most importantly, adjusts to changes in workload and schedules based on changing department and organizational priorities.
Successful candidate must possess the ability to function well under stress and is able to coordinate several tasks simultaneously while maintaining a high degree of efficiency and accuracy.
Minimum Qualifications:
Three to five years business/office operations and/or one to three years Hospice experience required
High school diploma or equivalent required; Associate's degree in a business related field preferred
Telephone related experience such as customer service representative preferred.
Two to three health care experience highly preferred
Strong proficiency in Microsoft Office products.
