Description:

Act as an advocate for each person served. Actively provide direct supports, case management and case coordination services and support to assigned individuals and programs. First point of contact for all things related to our Malden Office that houses our Individual Support Services. Develop, implement and monitor individual support plans and work in close collaboration with the ISP team. Manage bank accounts for all individuals receiving services through our Individual Support Program. Maintain regular contact with all stakeholders, uphold and support the organization’s mission, philosophy and values.

Job Description

  • Promote and protect the rights and dignity of each person served. Continually monitor support relationships for potential abusive or neglectful situations.
  • Manage the bank accounts of all individuals receiving services in the Individual Support Program. Go to the bank on an as needed basis to withdraw funds for designated individuals. Provide individuals with cash for spending over an agreed upon period of time.
  • Manage the personal spending budgets of all individuals receiving services in the Individual Supports Program after necessities like rent, utilities are paid for and groceries are purchased.
  • Maintain case records in accordance with CHS, Inc. policy and the regulations of the Department of Developmental Services.
  • First point of contact for all office related items; visitors, scheduling support, meeting support, office supplies, etc.

Requirements:
  • Ability to deal with a wide range of intellectual problems and practical problems
  • Ability to utilize excellent judgment in developing sound solutions
  • Combination of sitting, standing, and walking & must be able to lift 30 pounds
  • Ability to successfully perform the essential functions of this position with reasonable accommodations considered to enable individuals with disabilities to perform the essential functions
  • Personal commitment to service quality and excellence
  • Proven leadership skills and supervisory experience
  • Excellent verbal and written communication skills and demonstrated ability to work as a part of a team.
  • Knowledge of services and supports for people with developmental disabilities is preferred
  • Ability to read and write in English
  • Familiarity with computers
  • Bachelor's degree preferred
