Location: Hybrid; this position will require some in person work such as meeting with your caseload. Our main office is in Lafayette Colorado and we serve families in Boulder County. Only candidates who live locally will be considered.

Purpose of Position- Works with Spanish speaking or English speaking families of children ages 0-3 to access and navigate early intervention services, by providing Service Coordination to an assigned case load. Develops and implements an IFSP. Coordinates and monitors the implementation of services/supports written in the Plan. Assists families in acquiring services and supports. Maintains regulatory compliance; this includes ensuring documentation is completed to obtain and maintain ongoing funding for services for children and families on case load.

Essential Duties/Responsibilities
  • Coordinates delivery of services and supports.
  • Advises families on appropriate service options and assists in acquiring needed services/supports
  • Coordinates the delivery of services and supports from Imagine! and other organizations providing services/supports to the individual and/or their family.
  • Utilizes professional discretion in consulting with providers to ensure best practices in service delivery.
  • Assists individuals and families in resolving complaints with service providers and benefit sources. This may include the formal appeals process.
  • Facilitates exits and follow-up services for children and families as needed or requested; includes ensuring that proper notice of discharge is provided.
  • Provides support and resources to families as needed.
  • Mediates for, and negotiates with, IFSP team members and service entities to ensure congruent and effective service delivery for clientele.
  • Contacts families on a monthly basis and documents the contact in the main file.
  • Completes Targeted Case Management billing on a monthly basis.
  • Ensures that all required documentation is in accordance with applicable Federal, State, and Imagine! rules and regulations.
  • Ensures that master file for each individual is maintained in accordance with applicable rules and regulations
  • Ensures the completion of paperwork required to obtain and maintain availability of funding for services/supports for each person on the caseload.
  • Responsible for the development and implementation of the IFSP.
  • Leads the IFSP team in both periodic and annual reviews in the maintenance and development of the IFSP for clientele. Ensures written document is completed and distributed. Facilitates communication throughout the year regarding IFSP and related services and supports.
  • Monitors services throughout the year to ensure alignment with the current individual needs and the IFSP.
  • Ensures that the IFSP team reviews and revises the IFSP, as necessary, on at least a six month and a yearly basis, or more often as needed by the family or the team.
  • Acts as an interpreter and translator as needed for Early Intervention.
  • Helps Spanish-speaking individuals connect with English speaking Imagine! employees in various program areas that may work with the Service Coordination team.
  • Translates documents as needed that relate to the needs of the families in the Early Intervention department.
  • This position requires regular use of a personal vehicle with liability insurance that meets Imagine! standards.
  • This position requires regular use of a personal mobile device such as a smartphone or tablet.
Other Duties/Responsibilities
  • Advocates for the development of services/supports needed, but not available, for people with developmental disabilities and/or their families.
  • If position occupant has M.S.W., may supervise or assist in supervision of social work interns placed at Imagine!.
  • Other duties as assigned.

Job Qualifications

Knowledge, Skill, and Ability:
  • Fluency in Spanish and English. Prior to hiring, must pass a language test from The Language Testing Institute with a score of Intermediate High or above.
  • Solid skills in working with people with developmental disabilities and/or their families.
  • Solid independent judgment and discretion in monitoring, advising, and advocating for individuals and their families.
  • Solid skills in working with people with developmental disabilities and/or their families.
  • Solid independent judgment and discretion in monitoring, advising, and advocating for individuals and their families.
  • Solid knowledge of family dynamics.
  • Ability to work independently and prioritize tasks/goals for self and others.
  • Effective written and verbal communication skills with individuals and groups at all professional levels.
  • Strong planning and organizational skills.
  • Effective and creative negotiator and problem solver. This includes an ability to provide crisis intervention as well as mediate disputes.
  • Ability to work effectively with various technology including word processing software, internet based databases, etc.
  • Possession of a valid driver’s license and ability to meet Imagine! driving requirements.
Training/Education:
  • Bachelor’s level degree of education,
OR
  • five years of experience in the field of developmental disabilities, OR
  • some combination of education and experience appropriate to the requirements of the position
Experience:
  • Preferred experience working with people with developmental disabilities and their families, or experience working with young children and their families.
Working Environment/Physical Activities
  • Requires the ability to travel to various locations to meet with families, providers, etc.
  • Effective with shifting roles, responsibilities, and expectations in a highly stressful and changing work environment.
  • Ability to respond to emergency situations as-needed.
