Basic Function:
The Behavioral Health Care Manager provides assistance to the Behavioral Health Care Team. The Behavioral Health Care Manager is responsible for supporting and coordinating the mental and physical health care of patients.

Essential Functions:

1. Support the mental and physical health care of patients. Closely coordinate care with the patient’s Behavioral Health Care Team and, when appropriate, medical provider.

2. Works with front office staff to ensure appropriate behavioral health patient flow.

3. Escorts behavioral health patients from the lobby to the Behavioral Health department in a timely and efficient manner per Canyonlands’ current standard.

4. Conducts patient intake screening, including, but is not limited to: vital signs and chief complaint when appropriate.

5. Screen and assess patients for common mental health and substance abuse disorders. Facilitate patient engagement and follow-up care.

6. Provide patient education about common mental health and substance abuse disorders and the available treatment options.

7. Facilitate referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment).

8. Systematically track treatment response and monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.

9. Answer the telephone in a professional manner; direct calls to appropriate individuals and/or take accurate messages in the patients EHR, per standard.

10. Performs follow up calls to behavioral health patients as needed.

11. Maintenance tasks relating to the EPM system to ensure accuracy and current data; UDS, insurance financial information.

12. Document in-person and telephone encounters in the patient EHR.

13. Fax/copy information as needed. Performs other clerical duties as needed.

14. Participates in maintaining the MAT program.

15. Completes other medical assistant duties as assigned for the MAT program.

16. Complies with CHC Standards of Conduct, Code of Conduct and Corporate Compliance.

Secondary Functions:

1. Use PPE as appropriate.

2. Understands role in Disaster Plan and that safety is a condition of employment.

3. Follows CHC’s policies and procedures.

4. Serves on various CHC and community committees as required/requested.

5. Attends meetings, seminars, and conferences as appropriate to ensure skills and knowledge remains current.

6. Travel to other sites to provide coverage as needed.

7. Other duties as assigned.

Requirements:


Minimum Qualifications:

1. Ability to perform each essential function duty satisfactorily with or without reasonable accommodation. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

2. Completion of a Medical Assistant training program accredited by the CAAHEP, ABHES, ACCSC, or other accrediting body recognized by the US Department of Education or completion of a Certified Nursing Assistant program.

3. Possess a valid driver’s license.

4. Able to obtain and maintain a level I fingerprint card.

5. Knowledge of mental and medical health conditions.

6. CPR certified (BLS).

7. Ability to read, write and speak English.

8. Ability to effectively engage patients in a therapeutic relationship, when appropriate.

9. Good interpersonal and human relations skills are required. Must possess skills to empathize and care for a variety of patients, including low-income and diverse cultures and treat everyone (internal and external customers) with respect and dignity.

10. Ability to maintain confidentiality at all times in all situations and follow the laws and intent of the most current HIPAA laws and regulations and Canyonlands’ security and privacy policies.

Preferred Qualifications:

  • 1 year of directly related experience.
  • Bi-lingual (English/Spanish/Navajo) language skills
  • Knowledge of Microsoft programs with the ability to effectively communicate using Outlook.
  • Knowledge of NextGen.
