In the role of ADRC Professional you'll assist our clients by providing information and assistance, assessment, benefits related services, wellness/prevention activities, eligibility functions, and/or long-term care options. This is a full-time, salaried, position working primarily Monday-Friday 8:00am-4:30pm.

Salary Range: $55,619 - $71,510

Uses motivational interviewing techniques to clarify opportunities to best meet customer objectives. Strives to meet customer satisfaction of ADRC services; identifies to supervisory staff opportunities to improve ADRC services, county services, and community services.

Receives ADRC customers via phone, mail, computer, walk-in, or other means. Follows ADRC process for interviewing, documenting, providing service, making referrals, coordinating with service agencies, and following up with customers.

Utilizes ADRC tools such as client database, resource database, resource guides, pamphlets, technology, and documents (service agreements, release of information forms, referral forms, etc.) effectively.

Coordinates daily schedule to meet the needs of the section, taking into account client appointments, office coverage needs, training or other meetings, and time needed away from work. Communicates plans to modify daily schedule with appropriate notice to supervisory staff.

Explains and discusses options with clients regarding private pay and publicly funded options to meet their needs. Displays a thorough knowledge base of community and public programs and services to customers.

Makes community-based visits to assess client's needs/ability to pay for or the need for public assistance and offers the client services and/or program options as needed. These out of office visits may include visits to personal residences, medical facilities, schools, assisted living facilities, and other community locations.

Provides information and assistance to consumers, families, caregivers, and other professionals regarding services per Wisconsin Statutes, and/or requirements of the program, state, or federal guidelines.

Makes appropriate internal and external referrals for service.

Explains and discusses the client's rights and responsibilities in relation to ADRC services and those of public programs.

Maintains case records containing pertinent, accurate and current information.

Prepares correspondence, reports and other records as required.

Fully completes required documentation in a timely fashion and in accordance with program requirements.

Coordinates or participates in care conferences, family meetings, or other client related gatherings.

Participates in committees and task forces as assigned.

Follows all guidance related to conflicts of interest by making supervisors aware of issues as they arise and responding in accordance with mitigation options or by ending activities that conflict with the ADRC needs, as defined by policy.

Utilizes supervision time appropriately to discuss case issues, identify client safety (abuse, neglect) concerns, fraudulent activity, and issues or errors in casework so they may be proactively mitigated.

Maintains confidentiality of client-related information, maintains respectful treatment of clients, and adheres to the La Crosse County Standards of Conduct in the Administrative Code (3.08).

Maintains skill and competence levels as policies and regulations change by attending department/staff meetings and by participating in continuing training as required and appropriate. Follows required state training guidelines pertinent to the duties assigned.

Knowledge, Skills and Abilities

  • Basic understanding of the aging process including dementia, physical and developmental disabilities, mental health issues and substance abuse, local socio-economic issues and their effect on individuals and families, and family and informal caregiver needs.
  • Basic understanding of social systems, culture, diversity, family dynamics, and economic indicators of personal ability to achieve personal needs/outcomes.
  • Thorough knowledge of Medicaid, Medicare, Supplemental Security Income, Social Security Disability Insurance, Social Security Retirement, prescription drug assistance and various other financial assistance programs as well as IRIS and Family Care.
  • Ability to spot issues that create difficulty for consumers in navigating systems and accessing services.
  • Ability to relate appropriately to consumers/clients, family members and service providers.
  • Ability to conduct thorough individualized needs assessments.
  • Knowledge of the philosophy, history and development of social welfare programs.
  • Basic knowledge and understanding of human growth and behavior.
  • Knowledge of laws, regulations and practices pertaining to federal and state public welfare programs. This knowledge includes a thorough understanding of the functional and financial aspects of eligibility for public programs.
  • Knowledge of the planning needs for individuals who use private resources for accessing services available in the community that are commonly needed by individuals who are aging or who have disabilities.
  • Knowledge of resources for community welfare and health, and ways in which these resources may be used by people in need.
  • Ability to work with diverse populations.
  • Ability to relate to people in an unprejudiced and understanding manner with concerns for their circumstances and feelings.
  • Ability to communicate clearly and effectively orally and in written form.
  • Ability to use computer, email, internet and certain computer software programs.
  • Ability to work as part of a service delivery team and system in a positive, effective manner. This includes positive and effective interactions with consumers, co-workers, agency partners, state staff, advocates and others encountered in the work environment.

  • Knowledge of Chapter 51, 54 and 55 of the Wisconsin Statutes and the system in place that supports these laws.

Training, Experience and Other Requirements

  • Requires a Bachelor of Arts or Science degree (preferably in Social Work or another health or human services related field) or a license to practice as a registered nurse in Wisconsin pursuant to s. 441.06 Stats, and the equivalent of at least one year of full-time experience in a health or human service field, working with one or more of the client populations served by the Aging and Disability Resource Center (elderly or adults with physical or developmental/intellectual disabilities and/or people with dementia and family/informal caregivers). Qualifying work experience may be paid or unpaid and may include internships, field placements and volunteer work.
  • Waivers for the education and experience requirements may be granted based on the candidate's ability to fully perform ADRC responsibilities (as reflected by post-secondary education, experience, knowledge and skills. Exceptions may also be approved to enable the ADRC to employ individuals with disabilities and/or staff who are bi-lingual. Waiver approval may be conditional on the person's developing the knowledge and skills needed to fully perform all required job responsibilities within a specific timeframe, together with his/her on-the-job performance. The process for granting waivers is at the discretion of ADRC management.
  • Requires basic computer keyboarding skills for entering data related to clients
  • Must have valid driver's license and access to personal transportation, or other reliable means for daily city and rural travel.
