Flexible Schedule: Monday - Sunday (Weekends are Required) 8am-5pm

Summerfield Post Acute Rehab is a preferred provider of POST ACUTE REHAB services in Santa Rosa, CA. We have a solid reputation in the community with a long history of success and we need the best of the best to join us on our pursuit of excellence. We are currently looking for an enthusiastic and motivated individuals to fill some key administrative positions at our facility, Front Desk Receptionist and Admissions Coordinator.

Front Desk Receptionist

As the Front Desk/Medical Assistant you are the face of the facility and the first point of contact when any patients, visitors, physicians, and other guests enter the front door. We rely on you to provide a positive first impression and set the tone for their overall experience. The ideal candidate must have a natural ability to build instant rapport with those they meet while maintaining high standards of professionalism at all times. They also need to have strong communication skills and professional phone etiquette. Attention to detail and strong organizational skills are a must. We need a self-starter, someone who can work through issues with little guidance and supervision. This is a busy facility so we'll need someone who enjoys staying busy and thrives in a fast paced environment.

Duties:

- Greet all patients and visitors
- Operate a multi-line phone system and answer questions or direct callers to the appropriate department
- Assist all departments will clerical and administrative tasks as needed

Experience:

- At least 1 year of clerical, administrative, or customer service preferred.
- Experience in a healthcare setting is a plus.

Admissions Coordinator

Patient Admissions - The Admission Assistant, under the supervision of our Director of Admissions and Marketing Director will be responsible for reviewing and responding to inquiries and hospital referrals and converting them into admissions. You will be responsible for determining if a patient is appropriate for therapy in our facility and coordinating the patient admission with hospital Case Managers by obtaining proper admission orders, communicating with patients and/or their families, and facility staff for appropriate follow up. You will obtain patient medical records and patient medical information from hospitals, doctor's office, etc.

Customer Service -Assist patients and families in adjusting to the new environment of the facility by working closely with all departments. Assist families and patients in understanding policies and procedures of the facility and charges made for services rendered. Responsible for following up with patients and families to ensure they are satisfied with the service. Provide education to staff regarding effective customer service practices and lead facilities efforts to ensure a positive experience for all patients.

The ideal candidate will have prior experience in the following:

The ideal candidate will have prior experience in the following but we are willing to train the right candidate:

  • Healthcare (minimum 1-2 years)
  • Understanding of medical terminology
  • Experience with outside patient care services such as; transport, in home care, hospice, etc.
  • Discharge planning
  • Experience with insurance coverage such as; Medical, Medicare, Partnership, etc.
