PROGRAM SUMMARY:

Healthworks Assertive Community Treatment (ACT) is a participant-led approach to improving care and outcomes for people who are experiencing homelessness or housing insecurity in Windham County. ACT is an intensive, integrated services and support model designed for people whose basic needs are not being met by existing housing, medical, and mental health services. The Healthworks ACT team is designed to deliver integrated services of the participants' choice, assist in making progress towards goals, and adjust services over time to meet participants' changing needs. The staff-to-participant ratio is small, and services are provided 24-hours a day, seven days a week, for services as they are needed.

The Healthworks team will be adaptable, promote access and have strong communication skills, not only with participants but with the team and community (ex. shelter milieu, tent sites,hotels). Healthworks ACT team members are expected to provide outreach to community members, who are enrolled in the ACT program and/or who are served by Groundworks. The team will value and advocate for physical and mental health access and autonomy, through an equity and justice lens, for all participants.


HEALTHWORKS ACT TEAM RESPONSIBILITIES:

  • Participates in daily and weekly treatment meetings as indicated by shift.
  • Collaborates with ACT team staff broadly and Individual Treatment Team (ITT) members specifically to provide comprehensive services and supports.
  • Provides rotating coverage of holidays and on-call overnight shifts.
  • Document all services provided in Electronic Health Record
  • Participate and complete mandatory trainings (ie. ACT 101 and Intentional Peer Support (IPS)

POSITION RESPONSIBILITIES:

  • Performs clinical assessments, determines client service needs, and develops treatment/service in coordination with ACT team.
  • Develops and maintains substance use group services, including monthly ACT substance use outpatient group that provides education treatment utilizing agency and ACT approved evidence based practices and after-care support for substance use clients.
  • Provides client-centered treatment utilizing various modalities including motivational enhancement, harm reduction, and integrated dual-disorder treatment to encourage abstinence and sustained recovery.
  • Assesses clients in need of inpatient residential/detox treatment in cooperation with agency substance use department. Coordinates admission and discharge from inpatient treatment.
  • Will apply and adhere to the Ethical Standards applicable to the Substance Use Disorder profession
  • Performs other duties as assigned by the immediate supervisor which are consistent with the position and are in compliance with agency policies and procedures

QUALIFICATIONS:

  • Certified Substance Abuse Counselor or Master’s degree in a relevant field with professional, clinical experience in the provision of substance use treatment or a combination of education, training, and experience indicating the possession of essential knowledge, skills, and abilities
